YOUR NEW ROLE:

Holstep Health (formally known as Merri Health and Banyule Community Health) is a new chapter in delivering high quality health carer and support services to our communities and is one of the largest community health services in Victoria.

The Holstep Health Carer Services Program delivers Carer services, reaching carers in Melbourne's Northern and Western catchment area under the Carer Gateway (funded by the Commonwealth Government) and Support for Carers Program (funded by Victorian State Government).

The Team Leader, Assessment and Planning, oversees assessment, goal planning, service coordination and referrals for carers receiving supports through the Carer Services Program. The role directly supervises a team of Assessment and Planning Officers and a Senior Assessment and Planning Officers, who in return has operational line management over Service Coordinators and Intake and Assessment Workers. The Team Leader, Assessment and Planning is pivotal in the delivery of quality carer services and plays an essential role in the leadership team for the program.

YOU WILL BE RESPONSIBLE FOR:

As a Team Leader, Assessment and Planning, you will:

  • Provide clinical and line management to a team of Assessment and Planning Officers and Senior Assessment and Planning Officers.
  • Offer clinical supervision and consultations for the team, support other teams as needed, and undertake urgent clinical intervention and triage.
  • Ensure the team meets targets, KPIs, and funding guidelines.
  • Oversee team operations, ensuring efficiency and adherence to best practices in assessments, goal planning, service coordination, and referrals.
  • Contribute to the management, strategic planning, and development of the Carer Services Program as part of the leadership team.
  • Support practice and process improvements while ensuring compliance with standards and legislation.

What You'll need to succeed:

  • Bachelor qualification in social work, psychology, nursing, allied health or appropriate equivalent discipline and experience.
  • Highly-developed skills and experience to lead varied, multi-disciplinary teams.
  • Very good understanding of conducting clinical assessments, person-centred goal planning, referrals and service coordination.
  • Sound knowledge of the community care sector.
  • Proven ability to foster and maintain relationships with internal and external stakeholders and to relate to and work effectively with a diverse range of individuals and communities.
  • Excellent organisational, problem-solving, interpersonal and communication skills
  • Sound financial management, budgeting, reporting and compliance skills.
  • Proficiency in the use of information and communication technologies.
