Your new role

This role is your chance to join the highly skilled HARP Complex Care service at Holstep Health to provide coordinated, goal orientated care to clients with complex respiratory conditions.

  • You will work closely with the respiratory department at RMH to help improve client's quality of life and reduce avoidable hospital admissions.
  • You will be encouraged to share ideas through quality improvement initiatives and supported with ongoing professional development

You will be responsible for

As a HARP Respiratory Nurse, you will be responsible for:

  • Respiratory specialist assessment and providing clinical care through education, pulmonary rehabilitation, and care planning
  • Work closely with clients, their carers, hospital clinicians and GPs to provide integrated client-centred care for clients with respiratory diseases in accordance with best practice guidelines.
  • Improve self-management and health outcomes of clients through provision of education on anatomy and physiology of respiratory conditions, symptom recognition, medication, effective use of inhalers and spacers, breathing technique, exercise and exercise tolerance, advanced clinical assessment and exacerbation management.
  • Collaborate with HARP team members to identify and address client's psycho-social, behavioural, and environmental needs and other needs associated with their complex conditions

What you will need to succeed

To be successful with Holstep HARP team you will go above and beyond to achieve great results for our clients, the organisation and yourself. We're looking for individuals with:

  • Registered Nurse Division 1 and a current registration with the Australian Health Practitioner Regulation Agency (APHRA)
  • Post graduate qualification in Chronic Respiratory Disease Care or equivalent experience
  • Demonstrated experience in client centered care through effective respiratory assessment, care planning, case conferencing review and escalation of care for clients.
  • Demonstrated ability to work autonomously and collaboratively as part of a multi-disciplinary team.
  • Strong organisational, planning, time management and prioritisation skills
