Graduate Physiotherapist- Optimal community-based role with clients funded under Commonwealth Home Support Program (CHSP) - Commencing April 2025

Your New Role:
The Graduate Physiotherapist role is funded by the Commonwealth Home Support Program
(CHSP).
The CHSP funds services for clients 65 years and older (50 and older for Aboriginal and Torres
Strait Islander people). This program is underpinned by a wellness approach, which focusses on
clients' strengths, is goal oriented and aims to maximise independence and autonomy. The CHSP
also embraces principles of reablement and restorative care which are time limited and goal
oriented approaches that help clients adapt to a functional loss and/or address a particular deficit.
The Graduate Physiotherapy Program is designed to promote career opportunities for
Physiotherapists in the Primary Care setting. It aims to develop skills, knowledge and networks to
help a new physiotherapist build a future career in community health. The program provides on
the job training, professional development and support to help the graduate advance their career.

You Will Be Responsible For:

  • Providing evidence-based individual assessment, and management for older adults presenting
    with a diverse range of geriatric and/or health conditions impacting their functional abilities. This may include musculoskeletal, orthopaedic, neurological, and other chronic health
    conditions.
  • Provide evidence-based management which may include such treatment options as exercise and movement-based therapies,education, functional mobility training, equipment prescription, falls prevention, health coaching and manual therapies.
  • Participation in planning, designing and implementation of group
    exercise and education programs.
  • Liaise with others involved with the management of physiotherapy clients including preparation of reports, progress reports and referrals to other disciplines

What You'll Need to Succeed:

  • Bachelor's Degree in Physiotherapy, or Masters of Physiotherapy (Graduate Entry).
  • Full AHPRA registration
  • Demonstrate the ability to provide quality, client-centred, evidence-based Physiotherapy services to older adults.
  • Demonstrate the ability to work effectively as part of a team with well-developed interpersonal and communication skills.
