  • Position: Supporting Connections Practitioner

  • Position Type: Fixed Term Part Time

  • Grade: SACS level 4

  • Part-time- 0.6 FTE/1-year contracted position: start date 21/7/2025 and end date is 31/7/2026

  • Location: Coburg

  • Working with residents of SRSs to identify and respond to unmet health and social needs

  • Working with proprietors and SRS staff to enhance their capacity and skills in supporting residents

  • Liaising and networking with other health and local stakeholders to improve accessibility and support for SRS residents and staff

  • Development of an annual Collaborative Plan with each participating SRS.

Your New Role:

We are seeking a passionate and experienced Supporting Connections Practitioner to work within Supported Residential Service (SRS) Support Program. This exciting role will provide a unique opportunity to work as part of a multi-disciplinary team, focused on providing capacity building and service co-ordination to designated pension-level SRSs.

You Will Be Responsible For:

  • Developing, implementing and evaluating client-centred and strength based intervention to address residents' unmet health and social needs, and complex behaviours

  • Provide high standard psychosocial, functional and risk assessments

  • Build the capacity of SRS staff through community development interventions such as community connections, information and capacity building workshops

  • Actively contribute to team initiatives, planning and coordination of program activities

What You'll Need to Succeed:

  • We are looking for individuals with:

  • Relevant tertiary qualifications in social work, community development or equivalent

  • Experience in the provision of services to clients with complex needs, including behavioural issues

  • A minimum of 3 years experience in the community sector

  • Ability to use initiative and work creatively in unstructured environments

  • High level of adaptability and the capacity to communicate effectively with a wide range of diverse stakeholders
