YOUR NEW ROLE:

Care Coordinators work with clients with a chronic condition and/or complex needs along with members of the multidisciplinary team to ensure that the clients have the most appropriate services available when required.

This role is a unique opportunity to be a part of a passionate and highly skilled team, focused on prevention of chronic conditions and facilitating health behaviour change.

Care Coordinators will achieve this by providing:

• client centred care that is culturally responsive.

• client-focused broad multidisciplinary assessment and appropriate care planning

• build client self-management capacity.

• support the client and significant others to determine their preferred care options and most appropriate pathways.

• early intervention to minimize adverse effects and promote ongoing health and wellbeing including working collaboratively with the Allied Health, Nursing and NDIS assessor team for best outcomes

The successful candidate will also be encouraged to innovate and share ideas through involvement in quality improvement initiatives and have opportunities to be involved in program-level and/or organisational level committees.

YOU WILL BE RESPONSIBLE FOR:

  • - Complete holistic client-centered comprehensive assessments with clients who may present with complex or multiple chronic health conditions

  • - Unique opportunity to work across our flagship services including the Chronic Pain Service, Diabetes Clinic, NDIS assessment team

  • - Support clients in identifying their needs, encourage steps towards taking an active self-management approach and linking them in with necessary services or supports

  • - Have a broad understanding of services available both within Merri Health and externally to assist in navigation of the health system

  • - Engage meaningfully with multiple stakeholders including GPs, other health care professionals, external services for clients to meet their health care needs

  • - Be involved with unique opportunities including GP engagement work, health behaviour change and outreach health prevention talks and seminars

  • - Effectively manage a caseload and actively communicate within the multidisciplinary team, including at case conference, to ensure coordination of services and facilitate discharge pathways.

  • - Actively participating in professional development to maintain and enhance clinical knowledge and technical skills relevant for the role

WHAT YOU NEED TO SUCCEED:

- Relevant tertiary degree in the field of Allied Health, Nursing or Social Work

- Full AHPRA registration or other relevant registration body

- Demonstrated experience in delivering client-centred comprehensive health assessments

- Knowledge and experience of working with clients with complex needs and/or chronic disease management

- Excellent interpersonal skills and ability to work effectively has part of a team

- Strong organisational, time management and planning skills

- Keen interest in working with the Chronic Pain Service, Diabetes Clinic and other relevant programs

- Interest in developing health coaching and/or motivational interviewing skills

- Previous experience working in a community health or rehabilitation setting, hospital and outpatient settings or other community based organisation.
