YOUR NEW ORGANISATION

Holstep Health (formally known as Merri Health and Banyule Community Health) is a new chapter in delivering high quality health carer and support services to our communities and is one of the largest community health services in Victoria.

The Holstep Health Carer Services Program delivers Carer services, reaching carers in Melbourne's Northern and Western catchment area under the Carer Gateway (funded by the Commonwealth Government) and Support for Carers Program (funded by Victorian State Government).

YOUR NEW ROLE

The Carer Coach supports carers to improve their health and wellbeing through in-person carer coaching, following a coaching facilitation framework. Carer Coaches will deliver up to six sessions of coaching through a variety of modes, including face-to-face, telephone and online.

What You'll need to succeed

· Tertiary qualifications in social work or equivalent discipline and experience.

· Sound understanding of coaching principles and approaches including wellbeing coaching, motivational interviewing, solution focused coaching and person-centred approach.

· Experience delivering coaching in the community/health sector.

· Sound knowledge of the community care sector.

· Excellent organisational, problem-solving, interpersonal and communication skills.

· Proficiency in the use of information and communication technologies
