Brief Description:
Holstep Health is seeking an experienced Carer Supports Lead to oversee the delivery of high-quality services to carers across North and West Metropolitan Melbourne.

YOUR NEW ROLE:
Reporting to the Manager, Carer Services, the Carer Supports Lead will be responsible for managing the service delivery functions of Holstep Health's allocation of Victorian Carer Gateway funding, and the Victorian Government-funded Support for Carers Program across North and West Metropolitan Melbourne.
In this leadership role, you will oversee the delivery of high-quality, person-centred services, ensuring compliance with Commonwealth and State funding requirements, achievement of program targets, budget management, and strong quality, risk and clinical governance practices.
You will lead a multidisciplinary team of up to five people leaders, who in turn support the broader Carer Supports team. The successful candidate will bring strong people leadership, program management and stakeholder engagement skills, with the ability to manage service performance, budgets, workforce planning and complex client matters.
YOU WILL BE RESPONSIBLE FOR:
• Ensuring the effective and efficient delivery of assessment and planning, carer counselling, carer coaching and peer support under the Commonwealth Carer Gateway / ICSS funding guidelines and other relevant activities under the State Government Support for Carers Program Funding Guidelines.
• Ensuring services within the program are delivered in adherence to relevant Commonwealth and State guidelines (ie. Carer Gateway operating manuals, Holstep Health Standard Operating Procedures, Support for Carers Funding guidelines etc)
• Monitoring and supporting risk, quality and clinical governance requirements across the program.
• Effectively monitoring and managing carer, care recipient and community needs in service delivery and taking action to address identified issues.
• Meeting reporting requirements for Commonwealth funding and State funding, as directed by the funding guidelines/contract and the Director Carers
• Ensuring responses to complex client and carer issues, including resolving complaints and grievances, where required.
• Manage program finances including budget setting and ongoing maintenance, ensuring program operates within a responsible, sustainable financial framework.
• Ensuring the management of people within the team including recruitment, induction, professional development, reward and recognition and performance management
WHAT YOU NEED TO SUCCEED:
• Qualifications in community services / social sciences / health / management.
• Demonstrated experience (minimum 3 years) in leading, developing and implementing similar programs such as in aged care, carer support, and/or disability
• Proven track record in leading a highly engaged multi-disciplinary team
• Demonstrated experience in program planning and program evaluation.
• Highly developed written and verbal communication skills.
• Demonstrated experience in leading sustainable and effective change management.
• Track record in working in partnership with key stakeholders
• Experience managing program budgets and driving performance against KPIs
