Brief Description:

YOUR NEW ROLE:
Are you passionate about helping people stay active, independent and connected to their communities? As a Physiotherapist with Holstep Health, you'll provide evidence-based assessment, treatment, education and support to older Australians receiving services through CHSP and Support at Home funding.

Working within a supportive multidisciplinary team, you'll help clients maximise their independence, mobility and quality of life through services delivered in their homes and community settings. You'll enjoy the autonomy to manage your own caseload, with the flexibility to start and finish outreach days from home, supporting a healthy work-life balance.

Backed by dedicated support services that reduce administrative burden, you'll have more time to focus on delivering high-quality clinical care, collaborating with healthcare providers and achieving meaningful outcomes for your clients. YOU WILL BE RESPONSIBLE FOR:

* Delivering evidence-based physiotherapy assessments, interventions and education to support clients to improve mobility, function, independence and quality of life.

* Developing tailored treatment plans in partnership with clients, supporting goal setting and shared decision-making.

* Delivering individual and group-based programs and adapting interventions to meet changing client needs and circumstances.

* Managing a diverse community caseload, prioritising services to ensure timely and effective client outcomes.

* Working collaboratively with clients, carers, healthcare providers and multidisciplinary team members to deliver coordinated, person-centred care.

* Providing education and self-management strategies that empower clients to achieve their health and wellbeing goals.

* Maintaining accurate clinical documentation, reports and service records in line with organisational, funding and legislative requirements.

* Supporting Allied Health Assistants through appropriate delegation and supervision.

* Participating in professional development, clinical supervision and quality improvement initiatives to support continuous learning and service excellence.

* Contributing to a positive team culture and upholding Holstep Health's commitment to high-quality, evidence-based care.


WHAT YOU NEED TO SUCCEED:
* Approved qualification in Physiotherapy

* Current registration with AHPRA

* 5+ years' experience providing physiotherapy services in community, rehabilitation or clinical settings

* Strong clinical assessment and problem-solving skills

* Highly developed interpersonal and communication skills

* Ability to work both autonomously and within multidisciplinary teams

* Strong organisational and time-management skills

* Ability to work with diverse populations which may include people with disability, chronic illness and, children and older adults

* Cultural awareness and ability to work with diverse communities
