Brief Description:
Permanent, full-time role with hybrid arrangement available. This key role will help:
• Shape the future of aged care by leading Support at Home and Social Support Programs through an exciting period of reform.

• Lead high-performing teams across home care and social connection programs, empowering staff to deliver exceptional care.

YOUR NEW ROLE:
The Manager will guide the development of services within the program in an integrated way, identify and manage risks and engage and communicate with internal and external stakeholders.
Key roles include:
• Effective management, development, implementation, delivery and evaluation of all operations in the context of a changing external environment and fiscal constraints;
• Drive integrated and consumer directed care approaches with a focus on wellness, independence, restorative care and reablement;
• Develop and foster an effective and empowered work culture; and
• Strengthen collaborative partnerships to grow services.
YOU WILL BE RESPONSIBLE FOR:
Key responsibilities include:
• Provide effective leadership and operational management and direction across program areas
• Review current models of care, undertake gap analysis and the necessary change processes to embed evidence-based models that reflect contemporary thinking in relation to aged care directions
• Ensure well integrated models of care across Holstep's Aged Services and demonstrate effective collaboration in program design, delivery, monitoring and reach across Holstep Health and in conjunction with the Manager of CHSP Allied Health Services.
• Drive key projects to meet organisational and program requirements on time and within budget
• Provide subject matter advice on matters relating to aged care reforms, funding, and growth.
WHAT YOU NEED TO SUCCEED:
• Demonstrated experience at a senior level position in leading, developing and implementing a complex range of Home/Community Care programs
• Tertiary qualifications in Nursing, Allied Health or community based aged care related home and social care services
• Demonstrated experience in human resource management and leadership with a focus on creating values based aged care and an ability to lead and manage change
• Demonstrated experience in creating value and promoting a collaborative culture/environment
• Demonstrated experience in interpreting and applying policy, legislative and regulatory requirements that underpin aged care services
• Highly developed written and verbal communication skills
• Demonstrated experience in promoting a collaborative culture/environment and effective stakeholder management
• Track record in securing business growth and development opportunities and financial management and problem-solving skills ensuring achievement of outcomes within resource limitations
