YOUR NEW ROLE:
This role is the chance to join a passionate and highly skilled multidisciplinary team. You will be supported and encouraged to innovate and share ideas as we continue to grow and develop the service.
Funded by the Commonwealth Home Support Program (CHSP), this community nursing (continence advisory) role involves providing expert assessment, education, funding applications, interventions and referrals to aid older adults in managing incontinence problems with a specialized focus on clients living with dementia and cognitive impairment. Services are delivered within a client's home environment.
The service is underpinned by a wellness and reablement approach, which focuses on clients' strengths, and aims to maximise independence and autonomy. The role requires sound clinical reasoning, effective caseload management and a commitment to ongoing professional development and evidence-based practice.
YOU WILL BE RESPONSIBLE FOR:
- Providing high quality, evidence based services for all clients
- Undertake health and continence assessments using evidence-based practice including the identification of treatment options and promotion of self-management
- Provide education and advice to clients and their families regarding continence management
- Work collaboratively with clients (and carers where appropriate) to support shared decision-making, including identifying goals and developing care plans that reflect client preferences, values and choices
- Exercise sound clinical judgment to modify continence interventions in response to complexity, risk and changing client needs
- Completing timely and accurate documentation of all clinical interactions
- Actively participating in professional development to maintain and enhance clinical knowledge and technical skills and keep up to date with new developments and relevant trends.
- Participate in service development and quality improvement activities

What you'll need to succeed:
• Degree qualified in nursing and registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Division 1 Registered Nurse
• Demonstrated experience in providing high quality clinical continence care to older adults
• Good understanding of dementia-related issues and a desire to work with people living with dementia
• Excellent interpersonal skills and highly developed communication (written and verbal) skills
• Strong organisational, planning, time management and prioritisation skills
